1. Acceptance of these terms
These Terms of Service form an agreement between you and OCD Anxiety Centers Colorado LLC regarding the use of this website and the operation of our intake process. By browsing the site, sending a message through the contact form, calling our telephone number or emailing the intake desk, you confirm that you have read and understood these terms.
If you are using the site on behalf of another person, you confirm that you are authorized to do so. If you are contacting us about a minor, you confirm that you are the parent or legal guardian or that you have permission from the parent or legal guardian.
Clinical services are also governed by separate intake documents, including a consent for treatment and a notice of privacy practices. Those documents are presented before treatment begins, and where they conflict with this page, the signed clinical documents govern the provision of care.
2. Description of services
OCD Anxiety Centers Colorado LLC provides structured outpatient care for obsessive-compulsive disorder and anxiety conditions. Our published services include intensive outpatient programs, assessment and treatment planning, individual therapy sessions, family education workshops, group skills training and step-down continuing care.
The specific services offered to any individual depend on clinical need, availability and the judgment of the treating clinician. A description on this website is informational and is not a guarantee that a particular program will be recommended or that a place in a program will be available at a particular time.
We may modify, suspend or discontinue a program where this is necessary for clinical, staffing or regulatory reasons. Where a change affects a scheduled service, we will contact affected clients as soon as practical.
Our clinic is located at 11260 S River Heights Dr, South Jordan - 84095-5119, United States (US). Services are provided at that location or through agreed remote arrangements where appropriate and permitted.
3. Not an emergency service
This website and our intake desk are not emergency services. Messages sent through the website are not monitored continuously, and a reply may take a business day or longer. Do not use the website to report a crisis.
If you are in immediate danger, if you are thinking about harming yourself or another person, or if you are experiencing a medical emergency, contact local emergency services without delay. In the United States this generally means calling emergency services or going to the nearest emergency department.
If you are a client of the clinic and you need urgent help outside business hours, follow the instructions in the safety plan agreed with your clinician. Those instructions take priority over anything described on this website.
4. Website content is not clinical advice
The articles, program descriptions, frequently asked questions and other materials on this website are provided for general information. They describe our approach and explain what treatment may involve, but they do not constitute clinical advice and are not a substitute for an assessment by a qualified professional.
Reading about a condition does not create a clinician relationship. A clinician relationship begins only after an assessment has been completed and treatment has been agreed in writing or through the intake process.
Individual circumstances vary widely, and a technique that helps one person may be unsuitable for another. Do not start, stop or change any treatment, including medication, on the basis of website content. Speak with a qualified professional who knows your history.
5. Eligibility and intake
We provide services to adults and adolescents. Where the client is a minor, a parent or legal guardian must be involved in intake and consent to treatment, and the young person must also be willing to take part in the assessment.
Intake is a process rather than an automatic right to treatment. It includes gathering basic information, completing an assessment and agreeing on a plan. We may decline to accept a person for treatment where the service requested is outside our competence, where the level of need requires a different setting, or where accepting the person would create a conflict of interest.
Where we decline treatment, we will explain the reason at a level that is helpful and, where we can, suggest alternative sources of care. We are not obliged to provide services to every person who inquires.
You agree to give accurate information during intake. Inaccurate or incomplete information may affect the safety of treatment and may lead to a revision of the plan or a decision to refer elsewhere.
6. Appointments and scheduling
Appointments are scheduled by agreement with the intake desk. A scheduled time is held for you once confirmed. We ask that you arrive or connect promptly so that the full session can be used.
Clinicians may need to reschedule an appointment because of illness, an emergency or another clinical obligation. Where this occurs, we will offer an alternative time as soon as practical. We understand that rescheduling can be disruptive and try to keep such changes rare.
Programs such as an intensive outpatient block have a defined schedule, and some sessions are group based. Participants agree to attend the full block where they have accepted a place, and to inform the clinic in advance of any planned absence so that the group plan can be adjusted.
Reminders may be sent by email or telephone. A reminder is a courtesy, and it remains the responsibility of the client to keep track of scheduled appointments.
7. Fees, billing and payment
Fees for assessment and treatment are explained before an appointment is confirmed. We aim to be transparent about the cost of each service, including any program fee, session fee or materials charge.
Where insurance is used, we will explain what we can verify about coverage and what remains the responsibility of the client. A verification is not a guarantee of payment, and clients remain responsible for amounts that an insurer declines to cover.
Payment is due according to the terms stated in the intake documents. Where an account becomes overdue, we may pause scheduling and discuss a payment arrangement. We will not withhold access to urgent clinical records because of an administrative dispute.
Fees may change over time. We will provide notice before a change applies to an existing treatment plan, and clients may discuss alternatives or a referral if a new fee is not workable.
Questions about a bill should be sent to the intake desk promptly so that any error can be corrected. Our telephone number is +13188678091.
8. Cancellations and missed sessions
Appointment times are limited, and another person may be waiting for the slot you hold. If you cannot attend, please tell us as early as you can so that the time can be offered to someone else.
A late cancellation or a missed session may incur a fee, which is explained in the intake documents. We recognize that anxiety can make it difficult to attend, and we prefer to talk about the obstacle rather than simply charge for the absence. Please contact us if a pattern of missed sessions is developing.
Where a session is missed because of a clinical emergency, we will review the circumstances rather than apply a fee automatically. Where the clinic cancels a session, no fee applies.
Repeated missed sessions may lead to a review of the treatment plan, including a change in the level of care or a referral to a service that can better accommodate the schedule.
9. Acceptable use of the website
You agree to use this website lawfully and respectfully. You must not attempt to gain unauthorized access to the site, its servers or any connected system, interfere with its operation, or use automated tools to copy content at scale.
You must not submit content that is unlawful, threatening, defamatory or designed to harass staff. You must not impersonate another person or submit information about someone else without the authority to do so.
The contact form is intended for genuine inquiries about our services. Please do not use it to send advertising, chain messages or bulk requests.
We may restrict or block access to the website where we reasonably believe that these rules have been broken or that security is at risk. Where possible, we will explain the action and how to raise a concern about it.
10. Communication channels and security
Email and web forms are convenient but not perfectly secure. Please avoid sending detailed clinical information or sensitive documents through a first website message. The intake desk will arrange a suitable method for anything that requires additional protection.
If you receive a message that appears to come from our clinic but seems unusual, for example a request for payment through an unfamiliar channel or a link to an unknown website, do not respond. Contact us using the telephone number published on this site to confirm whether the message is genuine.
You are responsible for keeping your own devices and accounts secure when contacting a health service. Use a private device where possible and avoid shared email accounts for clinical correspondence.
We will never ask for your account password or full payment card details by email. If someone does, treat the request as fraudulent.
11. Intellectual property
The text, design, layout, graphics and other materials on this website are owned by OCD Anxiety Centers Colorado LLC or used with permission. They are protected by copyright and other laws.
You may view the site and print a reasonable number of pages for personal, non commercial use. You may share links to public pages. You must not copy, republish, sell or redistribute substantial portions of the content without written permission.
Names and marks used on the site, including the clinic name, belong to their respective owners. Nothing on this site grants a license to use those marks except as part of a permitted reference.
If you believe that material on this site infringes a right you hold, contact the intake desk with enough detail for us to review the claim. We will respond in good faith and remove material where that is appropriate.
12. Third party links and resources
The website may include links to external resources, professional organizations and public guidance. These links are offered for convenience and information. We do not control the destination sites and are not responsible for their content, accuracy or privacy practices.
A link does not mean that we endorse every statement on the destination site. Where a resource is referenced in clinical materials, the clinician will help you interpret it in the context of your plan.
If you notice a broken or inappropriate link, please tell us so that we can review it.
13. Disclaimer of warranties
This website is provided as it is and as available. To the fullest extent permitted by law, OCD Anxiety Centers Colorado LLC makes no warranty that the site will be uninterrupted, error free or free of harmful components, or that the information it contains is complete or current at all times.
We work to keep the content accurate and to correct errors when they are found. Nevertheless, clinical knowledge develops, and a page that was accurate when written may later need revision. The site is not a live clinical record and should not be relied on as one.
Nothing in this section limits any warranty or right that cannot be excluded under applicable law, including rights relating to the professional standard of clinical care.
14. Limitation of liability
To the extent permitted by law, OCD Anxiety Centers Colorado LLC is not liable for indirect, incidental, special or consequential loss arising from the use of the website or from reliance on its general content. This includes loss of data, loss of profit and loss arising from interrupted access.
This limitation does not apply to liability that cannot be limited by law, including liability for death or personal injury caused by negligence, for fraud, or for any liability arising from the professional duty of care owed to a client.
Where the website is unavailable, you may reach the clinic by telephone at +13188678091 or by email at intake@ocdanxietycenters.hair during business hours.
You agree that any claim relating to the website must be brought within the period permitted by law and in the forum described in the governing law section.
15. Indemnification
You agree to indemnify OCD Anxiety Centers Colorado LLC against reasonable losses, costs and expenses that arise from your misuse of the website, your breach of these terms, or your submission of unlawful content through the site.
This obligation does not apply to the extent that a loss results from negligence by the clinic or misconduct. It is intended to cover situations in which a person causes harm to the clinic or to another party through misuse of the website.
We will notify you of any claim that may trigger this section and will allow a reasonable opportunity to respond, except where doing so would compromise a confidential clinical matter.
16. Privacy and records
Our handling of personal information is described in the Privacy Policy, which forms part of the context for these terms. The Privacy Policy explains what we collect, how we use it, how we protect it and how you can exercise your rights.
Clinical records are maintained in accordance with professional standards and applicable law. Access within the clinic is limited to staff who need the information to provide care or to run the service safely.
Requests for records, corrections or other privacy rights should be sent to the intake desk. We may ask for identification before releasing records, and we will explain any legal limit that applies to a request.
Where these terms and the Privacy Policy appear to conflict on a privacy question, the Privacy Policy governs the handling of personal information.
17. Changes to these terms
We may update these terms to reflect changes in our services, in the law or in the way the website operates. The current version is always available on this page, and the effective date is shown at the end.
Where a change is material, we will provide notice on the website or by direct communication to clients where appropriate. Continued use of the website after an update indicates acceptance of the revised terms for future use.
Changes do not alter rights that have already arisen, and they do not apply retrospectively to an agreement that has already been completed.
18. Governing law and disputes
These terms are governed by the laws of the State of Utah and the applicable laws of the United States, without regard to conflict of law rules that would require the application of another jurisdiction.
Before starting formal proceedings, we ask that you raise the concern with the clinic so that we have an opportunity to resolve it directly. Many issues can be settled through a conversation about scheduling, billing or a clinical concern.
Where a dispute cannot be resolved informally, it may be brought in the courts of competent jurisdiction. Nothing in this section prevents either party from seeking urgent relief where that is necessary, or from exercising a statutory right that cannot be waived.
If any part of these terms is found to be unenforceable, the remaining parts continue in effect. A failure to enforce a provision on one occasion is not a waiver of the right to enforce it later.
19. Contact information
Questions about these terms, requests relating to the website and concerns about the service can be directed to our intake desk. We will acknowledge a written inquiry within a reasonable time during business hours.
